How do I return my completed patient feedback forms?
Once you have received all your completed forms, send pages 1 and 2 following each other.
You can post your completed pack, in one batch, and all your questionnaires in order to:

Please send your pack registered post if possible and to be 100% safe keep a copy of your work.

You can scan and email your forms to:
360@L2P.co.uk
(max 20MB attachment per email).
Please choose one or the other method, NOT both!
Duplicates will cause unnecessary delays in processing.
The 360@L2P.co.uk mailbox is dedicated to receiving completed questionnaires only.
Scanning:
- High Resolution only.
- Name of the Dr at the top of each page.
- Scan in A4 Portrait mode
- Page 1 and 2 of the questionnaires should always follow each other.
- Please name your file as this makes for easier searching if there is a query
Emailing the files:
- Send them all in one batch if possible. As we process large numbers of MSF every week, please avoid sending one 1 or 2 questionnaires at a time.
- Subject line: MSF forms for Dr XYZ - Email 1, Email 2, Email 3, etc. (if your files are too big to send in one email).
Please do not send photos of your MSF please as we will not be able to process them.
Once the L2P Processing Team has received your forms, the processing of your forms takes a few days. All forms received by Wednesday 1.00 pm, will be processed and uploaded to the system on Friday (the same week). All forms received after Wednesday's 1:00 pm cut-off will be processed and uploaded to the system the following Friday.
To receive your MSF report:
Please ensure you have provided at least the minimum number of responses as defined by your organisation.
Please ensure that you complete your Self Assessment.
If you have given us sufficient forms, the processing of your forms has gone smoothly and you have completed your self-assessment, your report will be released to your L2P account promptly.
